Chinese HIV-tests onbetrouwbaar waarschuwt Zuid-Afrika

De nieuwswebsite van de Verenigde Naties, IRIN-NEWS, schrijft deze week dat zo'n 1-miljoen Chinese zgn. 'Wondfo Rapid One Step Test kits voor HIV' door ZA uit hun testklinieken moeten worden gehaald. Ze krijgen in de klinieken onbetrouwbare testresultaten.De vervaardiger is gevestigd in Guangzhou, Guangdong Provincie in China.

De Chinese striptestjes bleken onbetrouwbaar te zijn toen ze door de organisatie New Start in twaalf drukke Zuidafrikaanse klinieken werden gebruikt door -- dikwijls ongetrainde -- personeelsleden. Ze waren wel vooraf goedgekeurd nadat ze intensief in de laboratoria van de Amerikaanse Federal Food & Drug Administration en de ZA gezondheidsdiensten waren getest maar er was blijkbaar geen rekening gehouden met de dikwijls zeer lage opleidingsniveaus van de mensen die de tests uiteindelijk moeten gebruiken.

Ook zorgwekkend nieuws is dat er in China, net als in Zuid-Afrika, ook al hoe meer HIV-geĆÆnfecteerde mensen een weerstand hebben ontwikkeld tegen de medicijnen die HIV bevechten.

Beide in Zuid-Afrika en China worden al hoe meer mensen gediagnoseerd met resistentie tegen medicijnen die ingezet worden tegen beide AIDS alsook Tuberculose. In Zuid-Afrika sterven er nu zo'n 350,000 mensen per jaar aan de combinatie van AIDS + resistente-TBC.

De directeur van het AIDS-instituut in Hong Kong, Chen Zhiwei, waarschuwt dat patiƫnten die met deze vorm van hiv besmet raken niet meer behandeld worden met de in China beschikbare medicijnen.

  • Volgens Zhiwei is de kans op verspreiding van het resistente virus groot, omdat patiĆ«nten in China lang niet altijd adequaat worden behandeld. In China is volgens Zhiwei een gebrek aan artsen en zorg op het platteland. Bovendien zijn de mensen onwetend, waardoor de kans op resistentie hoog is. Het virus kan ook ongevoelig worden voor medicijnen omdat patiĆ«nten de pillen niet volgens de regels slikken.

We moeten een manier vinden om de ziekte een halt toe te roepen, anders wordt het in China net als in Zuid-Afrika”, aldus Zhiwei. In Zuid-Afrika woont het grootste deel van alle hiv-patiĆ«nten ter wereld.

Hiv is het virus dat altijd leidt tot het dodelijke AIDS - acquired immune deficiency syndrome. Patiƫnten met hiv zijn ongeneeslijk ziek, maar door de hiv-remmers duurt het langer voor zij AIDS-klachten krijgen. In China leven zeker 700.000 mensen met hiv. In Zuid-Afrika, meer als 7-miljoen.

Helaas blijken de dikwijls onopgeleide gezondheidsmedewerkers in Zuid-Afrika te veel fouten met de Chinese test setjes te kunnen begaan in de drukke HIV-klinieken in Zuid-Afrika. Uit de 751 patiƫnten die een negatieve HIV-diagnose kregen met deze Rapid One Step Test Kits, bleken 19 mensen onlangs uiteindelijk toch wel HIV+positief te zijn.

De waarschuwing werd deze week gericht door Dr Dhayendre Moodley van de universiteit van KwaZulu-Natal's Vrouwengezondheid en HIV-navorsingsafdeling.

De ZA farmaceutische maatschappij FutureMed Pharmaceuticals verspreidt de Chinese vingerprik-test setjes in Zuid-Afrika. Deze maatschappij is geregistreerd op de Johannesburgse Effectenbeurs onder de naam Sekunjalo Health Care Limited -- en deze beursgang werd in 2005 gefinancieerd door Nedbank Capital. De directeuren zijn voorzitter Dr M. Iqbal SurvƩ (linksboven) en Mohammed (Mo) Kajee, besturend directeur, met Dominee Vukile C. Mehana van de ZA Methodisten Kerk als directeur. Het hoofdkantoor is Sekunjalo House in Tokai, Kaapstad.

Mirian Mhazo van New Start zegt dat de setjes beloven om al binnen dertig minuten een diagnosis te krijgen alleen maar van een klein vingerprikje, en de vervaardigers beloofden ook dat gezondheidsmedewerkers ze al konden uitvoeren met weinig of geen opleiding.

  • Dit blijkt dus niet waar te zijn in de overbezette Zuidafrikaanse gezondheidsklinieken. Zuid-Afrika heeft zo'n 8-miljoen HIV-geĆÆnfecteerde mensen en er komen er maandelijks vele tienduizenden nieuwe patiĆ«nten bij. De HIV-klinieken zijn dus bijzonder druk bezocht en er kunnen duidelijk veel fouten met zulke vingerprik-testjes gemaakt worden door de veelal minder-opgeleide gezondheidswerkers, waarschuwen experts.

Uit de 751 vrouwen die met deze bloedprikjes getest waren in de zeer drukke klinieken van KwaZulu-Natal dit jaar, bleken er 19 valse HIV-negative resultaten gemeld te zijn.

Dr Moodley van de universiteit van KwaZulu-zegt dat deze onbetrouwbaarheid van de snelle-testkitjes voor haar toch wel verrassend is. "De meeste van deze testkitjes waren immers vooraf getest en goedgekeurd in de laboratoria van de Food and Drug Administration van de VSA. Ook waren ze door onze eigen laboratoria geƫvalueerd en goedgekeurd.'

De HIV-experts begrijpen niet helemaal hoe de testjes zoveel valse resultaten konden opleveren tijdens het gebruik in de drukke HIV-klinieken in ZA.

  • Mogelijk gebruikten de gezondheidsmedewerkers de verkeerde volumes van het aktiveringsvloeistof of maakten ze andere fouten, zegt Moodley.

Als zulke testen uitgevoerd worden in laboratoria door opgeleide technici, moeten de instructies van de vervaardigers tot de kleinste details nagevolgd worden, en dit wordt mogelijk niet altijd gedaan door het veelal ongetraind personeel in de klinieken, die dikwijls ook veel haast hebben.

Zij beschreef het probleem in het Zuidafrikaans Medisch Journaal (SAMJ), waarin ze ook de aanbeveling deed dat de gezondheidsmedewerkers in de klinieken veel beter opgeleid moeten worden voordat ze de testkitjes mogen gebruiken.

Het heeft natuurlijk afgrijselijke gevolgen als iemand de verkeerde testresultaten terugkrijgt.
Mensen die geloven niet HIV+ te zijn zouden hun partners onwetend kunnen besmetten.
En een vals HIV+ resultaat "kan zeer ernstig emotioneel trauma tot gevolg hebben,' stelt zij.

"Wij verwelkomen zulke snelle testkits -- vooral omdat onze ZA laboratoria het zo vreselijk druk hebben. Maar wij geloven ook dat wij veel voorzichtiger moeten zijn in de selectie van zulke testsetjes. Ze moeten altijd eerst uitgetest worden tijdens het gebruik in onze klinieken, niet alleen in laboratoria, voordat er besloten kan worden welke merken aangekocht kunnen worden.'

"That endangered species, the South African white, now a mere 9% of the population and falling, while lumbered with a disproportionate share of taxes, discriminated against in the labour market and whose children face unfair hurdles to higher education, is now under attack from the -- self-proclaimed intellectual -- Xolela Mangcu.

IXolela Mangcun fact he calls himself a “public” intellectual, whatever that means. Maybe one can buy shares in him. His writings are generally incoherent and his latest rant particularly so. This is what he said and good luck in making head or tail of it:

  • Because they could put down blacks through law, white South Africans did not imagine they would not make the grade internationally. And so they continued talking about standards from a very low base...” (end quote).

Mulholland writes:

  • "No sane individual would deny blacks were repressed under apartheid. This was evil and shameful.
    "Not all whites supported it and many fought it in their own ways. Whites were beneficiaries whether they wished to be or not and reparations must be delivered to those formerly disadvantaged and dispossessed.
  • "But in so compensating for the sins of the past it would be self-defeating to drive away the skills which whites, and others, acquired in the past. Mangcu appears to be saying that SA whites do not measure up as their standards are at a “very low base”.

This flies in the face of all evidence, both anecdotal and statistical. Let me provide an example.

Today he runs the world’s largest orthopaedic hospital in Manhattan, which is part of Cornell University, where Mangcu obtained his doctorate in some sort of politics.

Even your aging correspondent, who failed to matriculate at the Durban High School, was head hunted to turn around Australia’s foremost newspaper publishing company.

There are numerous other examples, far more impressive than my own.

  • A major Australian law firm took a policy decision to have a hiatus on the hiring of South Africans.
  • All were so committed and talented that before long they were partners and this firm, one of the oldest and most distinguished in Australia, suddenly had a majority of SA partners.

An SA accounting qualification is among the most highly regarded in the world.

In Britain South Africans have risen to the top. They sit on the highest courts, are ennobled by the Queen, run vast businesses and work at the highest levels in medicine, accounting, engineering and architecture.

  • Sir Donald Gordon, founder of Liberty Life in SA and of Liberty International in the UK, is only one of a number of graduates of King Edward V11 High School in Johannesburg who have had spectacularly successful international careers.

In the US the graduates of the South African university of the Witwatersrand (Wits) have been a dominant force in one speciality, paediatric surgery.

  • These émigrés run large hospitals and departments at leading universities.

Greek-SA businessman Yianni Tyropolis slaughtered, Heuwelsig Bloemfontein

BLOEMFONTEIN, Heuwelsig. Oct 10 2008 - Beeld journalist Loraine Kalp writes that the well-known Greek-SA businessman Yianni Tyropolis, 62, was attacked so viciously that blood was spattered all over the walls and pooled on the floors of his home. Blood puddles pooled in the kitchen, dining room and the hallway of his house while he was being bludgeoned to death.

Tyropolis ran the Friendly Grocer shop in Universitas. He was ambushed inside his home when he returned yesterday.Police superintendent Susan Moseki of Bloemfontein said he was attacked by five men who spoke Sotho and Xhosa. Two domestic workers were also attacked, tied up and locked into various bathrooms. Tyropolis was forced into the house and immediately stabbed and bludgeoned by the men.

CARRIED OUT A HUGE SAFE

  • They then CARRIED OUT a heavy safe, containing firearms, ammunition, jewellery and some R100,000 in cash and drove off. They took nothing else of value from the house and left him behind, bleeding and helpless.

One of the maids got lose and alerted neighbours. Paramedics found upon their arrival that he was still alive. Mrs Amanda Appelgryn of the local Medi-Clinic said he suffered 'extensive damage to the skull' when he was brought in. The trauma personnel struggled for about half-an-hour to try and save his life - but he succumbed.

One hijacker dead, one injured - crashed through a wall in Lyndhurst

Oct 10 2008 André Damons of Beeld newspaper and Sapa report -- One of the two hijackers who had just attacked Jaco van Heerden, a 24-year-old Jeppe High School teacher died in a high speed chase through the streets of Lyndhurst north of Johannesburg this week. The two black hijackers had dragged the Afrikaans teacher from his car in his Lyndhurst driveway -- but then ran headlong into a wall during a high-speed chase. One unnamed hijacker has died and the other man is in critical condition in hospital after they had smashed through the wall, ending up in the garden belonging to Lyndhurst homeowner Mr Doug Hillen. Both hijackers had bullet-wounds as well as crash-injuries, police said.

The 25-year-old Jeppe High School teacher was confronted by the two armed men in his driveway on Thursday-morning just as he was setting off for school. The teacher, being consoled in the picture above, immediately alerted a passing car with two security guards -- who gave chase. Their vehicle also ended up against the garden wall, see picture above.

The driving hijacker then lost control of the car and ran fullspeed into a wall, crashed through it and came to rest in the garden of a family home.The security guards chasing them also ran into the wall, but managed to slow down in time and avoided serious injuries, said Lyndhurst police Inspector Hamilton Mazomba.

Lyndhurst, a well-protected middleclass suburb north of Johannesburg, also was the scene of a shocking crime which actually made the world headlines -- when a two-year-old child was killed execution-style during an armed robbery at her family home. Four armed men had broken down the property's palisade wall, broke down the burglar guards and smashed the lounge window to force entry. A woman who had just arrived from Nigeria with her baby from Johannesburg and was asleep in the bedroom, was confronted, and her money and cellphone demanded. The child woke up and started crying - so they just coldly shot the baby through the head to shut her up.

Wolf_Snarling Airport robbery gangs: Police believe that this ruthless gang was acting on insider information of the Nigerian woman's address through contacts at Johannesburg airport. In fact on Oct 10, airport security guard Steven Tshabalala was charged in Randburg magistrate's court for the murder of Qatar Airways operations manager Clay Pierre-Louis at the Rivonia street N1-offramp in July. His case was postponed to Oct 10. Pierre-Louis had only been in a country three months. His attackers shot him three times in the neck and head while he was talking to a SA friend to get directions - and only took his cellphone.  Two other people were also shot at that highway turnoff just recently and each time, they had been driving from Johannesburg Airport. A month earlier Cape Town journalist Megan Herselman of the magazine Drive Out was also shot dead in her rental car at this ramp. She had just landed at Johannesburg airport at around 19:35 from Cape Town and was ambushed enroute to a guest-house in Kyalami. Again, she was speaking on the cellphone with a friend, and again, she died in a hail of bullets from a police-issue 9mm handgun. They only took her cellphone. And only a week later, Mr Anver Mohamed, about whom almost nothing else is known, was also shot dead nearby, at the Glenhove-turnoff.

    The Herselman murder caused the launch of SA magazines You/Huisgenoot a "unite against crime petition" to then-president Thabo Mbeki and ex-security minister Charles Nqakula, who once told white South Africans in parliament to 'stop whinging about crime and get out of the country'...

High-security Centurion complex invaded: young couple in nightmare robbery

Oct 09 2008 - Midstream Security-Estates, Centurion, South Africa. These are Jaco and Samantha Muller of the high-security estate Midstream near Centurion.

Midstream security estates boasts of a double-line of security: first a 2.4m high palisade wall with an electric fence around the whole perimeter of Midrand Estates. The 2nd line is a 1.8m wall with an electric fence on top, which surrounds each individual estate. Between the double security walls an open area, 5m wide, patrolled 24 hours a day. Finger-print access control is installed at each estate's gate house with electronic monitoring systems and roaming, armed guards.

In spite of living behind this double-barreled security barrier, the young Afrikaans couple underwent a nightmare robbery on Wednesday-evening after returning from a pre-birthing class at 9.30pm. Mrs Muller is eight months pregnant.

Afrikaner livestock farmer Hansie Spies dies in hail of AK47 bullets

Oct 10 2008 HARRISMITH -- Dirk Kok and Jana van der Merwe report in the Volksblad, Bloemfontein -- Fifth-generation Afrikaner farmer Hansie Spies, 75, died in a hail of bullets from an AK-47 assault carbine on his family farm Gravelotte yesterday-morning. This brings our farm attack murder statistics to a total of 2,996 people killed since 1994.

  • And the only motive for murder, according to his sons George and Christo, must be their dad's formal opposition to a land-claim on a piece of land in Little Switzerland, in the Drakensberg mountains.

The Spies family has farmed in this region for the past five generations, they said. The sons farm with their dad on the farm Gravelotte.

Afrikaans girl Charmaine Pienaar, 18, left paralysed from farm attack

AttackedAfrikanerTeenParalysedFarmAttack Oct 9 2008 VEREENIGING, South Africa Beeld journalist Sonja van Buul writes that this pretty Afrikaans teen -- photographed during much happier times -- may never walk again. She needed 11 hours of intricate surgery after an October 1 2008 attack at her family's subsistence smallholding at Waldrift, Vereeniging, and which had also left her mom Heila dead.

Charmaine Pienaar, 18, needed surgery to her skull, fractured cheekbones and jaw, all splintered from heavy blows by the armed attacker who had also killed her mother on the family's subsistence smallholding. Apparently this attacker had 'promised not to hurt them if he just gave them the money.' After they did give them every cent they could find in the place, he still bashed the two unarmed Afrikaans women's brains in anyway.

  • He had cut through a 2,4-m electrified fence to reach them, just after the two Pienaar men had left to go and sell some scrap-metal they'd gathered.

CharmainePienaarMomDeadOct92008_BeeldThis white Afrikaans family -- like many hundreds of thousands of other whites in South Africa -- also is living a hand-to-mouth existence, surviving by gathering scrap-metal and other recyclable waste from the environment and reselling it to wholesalers -- so they can't afford to have more surgery needed for her at a private hospital. She was left helpless and bleeding for several hours next to her dead mom after the attack until she and her mother were found by her father and 21-year-old brother Frikkie.

The men had left to sell scrap-metal but had rushed back to the homestead after a family member warned that they couldn't raise the women on their cellphones. A family friend, Marius Vosloo of Meyerton, said the frail blonde girl is paralysed in both legs. The family can't afford to have more surgery which is needed for her, and which can only be undertaken by specialists at a private hospital. Just after the attack, Charmaine told her family that she'd had this 'terrifying dream in which she just wanted to protect her mommy...' Her mom was buried yesterday.

  • Yesterday she was taken off the life-support system for the first time and moved to a general ward. She's able to move her hands and arms - but not her legs.

Swedish, Australian tourists injured by speeding booze truck, Kruger Park

 KrugerParkLogo South Africa's worldfamous Kruger wildlife reserve has a crawling-speed law to protect even the tiniest  wildlife such as the environmentally-important dung-beetles  - but yesterday, a safari vehicle with seven foreign tourists was hit and overturned by a high-speed heavy liquor truck, going well over 80km an hour. Just minutes before, the same truck had just barely missed killing a warthog...

October 9 2008 --LOWER SABIE, Limpopo, South Africa. A wildly-speeding liquor-delivery truck hit an open safari vehicle, spilling and injuring sevenSwedish and Australian tourists in the famous Kruger National Park on Wednesday afternoon.

The Kruger -- the oldest wildlife park in the world -- has traditionally always maintained a 'crawling-speed' to protect the wildlife and to also allow tourists to view the animals without disturbing them overly much. However, the liquor truck with two men inside hit the safari-vehicle at a clip of between 80 to 90km an hour - spilling the seven tourists on top across the road and injuring them all.

The crash happened just a few kilometres from Lower Sabie - in the southern area of the Kruger -- after the driver of the Bushwise Safari vehicle, Mr Frank Pearmain, had stopped to show the tourists a herd of buffalo.  And nearby, there also were some elephants - not an ideal place to have an accident with tourists falling all over the road.

The liquor-delivery truck had sped straight at them and didn’t slow down. Pearmain says he got the impression that the truck driver was 'also trying to appreciate the wildlife, while driving at a "reckless" speed."

"We pulled over to the side of the road when we saw the buffalo on the left. On the right side of the road, occupants of another car looked at elephants and we thought that we’ll wait for them to leave to take a look for ourselves," said Pearmain.

  • Kruger_Elephant_Stroller_SabieThat was when they were hit by the heavy, speeding booze-truck from behind - flipping the safari vehicle over on its side and spilling the seven hapless tourists out onto the road.  Of the seven guests on the vehicle, everyone suffered minor cuts and bruises. One 60-year-old Swedish woman fractured her shoulder.

She, along with a 62-year-old Australian tourist, who lacerated his right hand, required treatment at the Nelspruit Medi-Clinic. According to a paramedic on the scene, the two occupants of the booze truck suffered a broken leg and possible neck injuries respectively. They were admitted to Shongwe Hospital. The local Skukuza police are not well-pleased: they are being investigated for reckless driving.

A doctor and paramedics were quickly on the scene and most of the injured were treated on site, while rangers kept a watchful eye out for roaming animals. As reported, there were herds of buffalo and elephants nearby....

"The truck hit us while travelling between 80 and 90 kilometres per hour. He must’ve been looking at the elephants as well, because he didn’t apply any brakes before the impact," said Pearmain. The safari vehicle was more 'clipped than being hit full-on, but it was enough to cause consternation.

"Someone on the scene told us that the same truck nearly hit a warthog a little earlier," he said before claiming that 90% of the speeding in the park is attributed to contractors.

However the Kruger's public-relations officer William Mabasa denies this, saying that 'speeding or reckless driving isn’t a major issue here. Obviously here and there someone will break the speed limit, but it isn’t a big problem. Saying that, if one person speeds, it’s one too many," he concluded. Those bruised and battered tourists probably wouldn't agree with him, however...
